  • Doctors Physical; scan & save a copy of the doctors physical on your computer, then upload for approval by the administration. Check your account to view when a new physical is needed.
  • Insurance: Enter the policy information
  • Permission Form (waiver), Athletic Contract and Concussion Information are available for viewing and acceptance upon checkout. You may also view them by clicking on the appropriate link below.
